29 CFR 1910.119(h)(3)(iv): The contract employer did not assure that each contract employee followed the safety rules of the facility including the safe work practices required by 29 CFR 1910.119(f)(4). On or about January 5 , 2019 at the Houston Refining facility at 12000 Lawndale St, Houston, Texas 77017, the employer did not ensure that employees affixed a personal lockout or tag device to a group lockout device before installing an Isolation Test Plug on a 16 inch crude oil pipeline.
