29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about January 09, 2019 employees were exposed to struck-by hazards where the employer did not provide powered industrial truck operator training to employees assigned to operate powered industrial trucks. On Friday, January 04, 2019 an employee was hospitalized following an incident in which the employee's foot was caught between a powered industrial truck and a battery charger when the untrained warehouse associated was backing a "Raymond" model 750-R45TT stand-up forklift and put his foot out to help slow the truck before impacting the battery charging station.
