29 CFR 1910.178(o)(6): Tilting forward with load engaging means elevated was not prohibited except when picking up a load: a) At the Area 4 reactor site, on or about and at times prior to January 4th, 2019, the employer did not prevent the tilting of totes containing triethylene glycol when elevated (approximately 24 inches from the ground level) by a powered industrial truck when being drained into a reactor via vacuum pump, thus exposing employees to struck-by hazards.
