29 CFR 1910.29(b)(2): The employer did not ensure that mid-rails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: The employer is failing to protect employees from fall hazards associated with tire storage. This was most recently documented on or about 01/03/19 at the establishment located at 3608 W. 30th St. South, Wichita, KS. The employer did not ensure that mid-rails, or the equivalent were installed, exposing employees to fall hazards at the following locations: a) Main Warehouse (Top floor) above the conveyor. The opening was approximately 56 inches in width and 38 inches high from the floor to the top of the guardrail and approximately 7 feet to the concrete floor below. b) Back East Warehouse along the staircase. The opening was approximately 34 inches in width, 37 inches in height from the floor to the top of the guardrail and approximately 7 feet to the concrete floor below.
