29 CFR 1910.23(b)(9): (9) Ladders were not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury: a) Building Exterior-Mechanic Shop: An employee was using an extension ladder that was not inspected before each use and more frequently as necessary to identify any visible defects that could cause employee injury. On 12/26/2018 an employee sustained head, shoulder, and hip injuries when he fell from a ladder that was not inspected prior of being used.
