29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training, verification and monitoring of de-energized equipment, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable: (a) On or about December 19th, 2018, for employees exposed to fire hazards while performing line breaking on process piping to the motor of a wellhead pump unit without controlling hazardous energy at or near, Well Site ELI 8-1E-163-102 in Fortuna, North Dakota.
