29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about December 19, 2018, and at times prior thereto, employees were exposed to a caught in between and crush hazard from utilizing a Nissan forklift (Model LP) with extended attachments that were not marked to identify the adjusted load capacity.
