29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about 12/17/2018, at a jobsite located at 2000 Park Avenue, Miami Beach, FL, the employer did not conduct adequate frequent and regular inspections of the job site to address fall, impalement, slip and trip hazards that existed due to missing guardrails, uncapped rebars, defective ladder, construction debris and lumber throughout the 5th floor deck. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
