29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On and before December 13, 2018, in the blast area of the Cleaning and Finishing Department, casting skids weighing 1 ton, measuring 300 inches long, 50 inches wide, 21 inches tall and stacked five high were not stacked, blocked, interlocked or limited in height so that the stack was stable and secure against sliding and collapse when: A) Casting skids were stacked on transfer-cars and holding rails; and B) Lateral loads and forces were imparted on the stacked casting skids causing misalignment, rotation, and collapse of the stacked skids while they were being moved on the holding rails or relocated by transfer car. The two independent pneumatic winches, located on either side of the holding rail, did not apply an even pulling force and the holding rails were not properly maintained, in that they were missing rollers, rollers were misaligned, and holding rail braces were bent.
