29 CFR 1910.212(a)(3)(ii): Point(s) of operation guards were not designed and constructed to prevent the operator from having any part of his/ her body in the danger zone during the operating cycle. On or about December 3, 2018, an employee received a right thumb amputation while operating a Relay Line Bender Machine inside the Relay Department and the employer did not ensure proper guarding was installed to eliminate amputation hazards near the point of operation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
