29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Margarita Orta, DBA Professional Roofing Crews at 982 St Andrews Lane, Louisville, Colorado: On or about 11/29/2018 the employer failed to ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Four employees were working on a two-story residential roof; three were not wearing a harness or utilizing any other fall protection. One of the employees fell two stories to the ground, resulting in multiple fractures. This condition exposed employees to fall hazards.
