29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented where the employer required respirator use. The employer required employees to wear respirators when spraying isocyanate insulation, but the employer did not develop a written respiratory protection program that described how the employer would provide training and other protective measures for employees, including: a) Information on how to select appropriate respirator b) Medical evaluations to determine employee's fitness for wearing a respirators c) Training on what to do in case of emergencies d) Information on use and maintenance of respirators e) Information on the proper procedures to follow in order to ensure that employees understand the importance of the quality, quantity, and flow of air when wearing atmosphere-supplied respirators f) Training on the hazards to which the employees will be exposed g) Procedures for employer to regularly evaluating the effectiveness of the program
