29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. Located at 289 Elm Street, Marlborough, MA On or about November 26, 2018, the workplace fatality was not recorded on the OSHA 300 form within seven days on the OSHA 300 Form or equivalent for the calendar year 2018. On or about November 7, 2018, an employee received a contusion on the head from bumping into the rear roll-up door of a box truck and died eight days later on November 15, 2018, due to blunt head trauma and a left epidural hematoma.
