29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval. a) 4th floor: A forklift stacker was used with a shorty lift boom attached to the fork blades for moving and installing wall panels. But the capacity, operation, and maintenance instruction plates, tags, or decals were not changed before using the affected forklift. The addition of using a lift boom affects the lift capacity and safe operation of the forklift stacker, and the lift boom was used without the forklift stacker manufacturer's written approval.
