The employer is failing to maintain OSHA Injury/Illness records, also known as OSHA 300 Logs, to document any and all recordable and reportable injuries and illnesses at this work location. The employer did not provide injury and illness logs to OSHA within 4 business hours. The employer was asked for the documents on different occasions and the documents were not provided OSHA.
