29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Professional Business Solutions of Western Colorado, Inc. at 2500 County Road 26, Rifle, CO: On and before November 13, 2018, the employer failed to report within 24-hours a work-related incident resulting in in-patient hospitalization. The injured employee worked for Professional Business Solutions of Western Colorado, Inc., a temporary staffing agency, but was under the supervision of host employer Mesa Completion Services, LLC, when the injury occurred on November 10, 2018. The injured employee tripped and fell from a frac tank storage tank, while climbing onto the tank. The employee fell approximately 9 feet and 4 inches and suffered injuries, resulting in in-patient hospitalization. The injury and in-patient hospitalization was not reported until November 14, 2018.
