29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employer does not protect employees from falls by providing necessary fall protection. The most recent incident of such failure occurred on November 16, 2018, at 11141 Spinner Ave, Sharonville, a church and school building, five roofers were exposed to a 25 ft. fall hazard on a 6/12 pitch roof while removing shingles and installing new shingles without utilizing any fall protection. Hernandez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1926.501(b)13), which was contained in OSHA inspection number 1297758, citation number 1, item number 1, and was affirmed as a final order on June 19, 2018, with respect to a workplace located at 9040 Debbie Drive, West Chester, OH 45069. Hernandez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29CFR1926.501(b)(13) which was contained in OSHA inspection number 1078571, citation number1, item number 1 and was affirmed as a final order on October 5, 2015, with respect to a workplace located at 11036 Embassy Dr. Cincinnati, OH 45240. Hernandez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1926.501(b)(13), which was contained in OSHA inspection number 1058419, citation number 2, item number 1, and was affirmed as a final order on July 20, 2015, with respect to a workplace located at 4834 Prosperity Pl, Cincinnati, OH 45238.
