OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards due to the improper installation of a ladder grab bar device on a roof hatch ladder: Location: Building B, roof hatch fixed ladder On or about November 8, 2018, an employee attempted to descend down a roof hatch fixed ladder and fell approximately 18-feet to the concrete floor below, due to a ladder grab bar device installed incorrectly on the top two rungs and face of the ladder.
