29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Jay Industries, Inc. - Mansfield, Ohio: On or about November 6, 2018, the employer failed to ensure that the point of operation on the Wysong #611 brake press (model H-3572) was guarded to protect employees during press operation. Employees that performed tasks such as, but not limited to, loading and unloading parts were exposed to amputation injury.
