29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Screw Department: On, or about, October 30, 2018, an employee suffered a partial finger amputation while changing a tool insert on Machine #163, a Model TUR 560 Horizontal lathe. The machine was not locked out in accordance with the Energy Control Procedure nor were alternative protective measures used to provide effective employee protection from the movement of the saddle fixture tooling and components.
