Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about October 29, 2018, the employer failed to ensure that employees conducting roofing activities on the flat roof of the building locate at 1821 Northeast Ave., Panama City Florida, were protected from falling 18 feet to the ground by the use of personal fall arrest system, guardrail system or safety nest system.
