29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) This violation occurred on or about October 26, 2018 at the straw-line area, at straw line one, where an employee clearing a plastic jam from the die was exposed to a crushed-by hazard when the machine unexpectedly started. In the alternative: 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: (a) This violation occurred on or about October 26, 2018 at the straw-line area, at straw line one, where an employee clearing a plastic jam from the die with the corrugating machine in the open position was exposed to a crushed-by hazard when the die closed.
