29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about October 29, 2018, employees were exposed to a fall hazard gap opening of approximately 30 inches high by 4 feet wide and 8 feet long. The employer did not ensure milk delivery driver employees utilized fall protection while sanitizing the internal walls of the milk tankers from the top of the tankers. The milk tankers were approximately 13 feet above ground level with an open sided edge stationary platform measuring 24 inches by 30 inches positioned at the top and next to the tanker hatch cover. The milk bay cage around the tanker platform and hatch cover was a rectangular railing system that was approximately 4 feet wide by 8 feet long and 4 feet high. The cage system did not have a platform base and was completely open at the bottom allowing an unprotected 4 foot by 8 foot by 30 inch high gap between the tanker top and the bottom rail of the cage.
