29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about October 31, 2018, and times prior, at the workplace located at 700 E. Jones St., Milwaukee, WI 53207, D&D Building #258, 2nd floor, the employer did not ensure employees implemented energy control procedures prior to sub-contracted employees entering the drum of Dryer #5 to perform blade replacement operations. The machine was not isolated from all energy sources prior to sub-contracted employees engaging in these servicing and maintenance activities. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from the unexpected energization, startup, or release of stored energy during machine servicing and maintenance activities.
