29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: (a) On or about October 15, 2018, employees were involved in a forklift accident. The employer did not ensure that the forklift operator was retrained after the accident. This exposed employees working around the forklift operator to struck by hazards.
