29 CFR 1910.147(c)(6)(i)(C): Where lockout is used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected: a) On or about October 23 and 24, 2018, on Line 43, employees were required to service and/or maintain the Gabler Machine and/or its associated equipment. The employer did not adequately conduct and certify an annual or more frequent inspection of the energy control procedure for the Gabler Machine that included a review between the inspector and each authorized employee of that employee's responsibilities, under the energy control procedure being inspected, to ensure that the procedures and requirements of this standard were followed. The employees were thereby exposed to crushing hazards associated with moving parts.
