29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) Northeast Precast LLC.- Throughout the manufacturing plant: The energy control procedures were not specific for each individual type of machine including but are not limited to: Geka Bendicrop 85 SD Ironworker machine, TJK Rebar cutting machine, Gensco Table bending machines, CBP 1200 Press, Voeller Mixer, etc. The program only has a one fits all LO/TO procedure and it did not clearly and specifically identify the placement, removal or transfer of Lockout /Tagout devices on each individual type of machine and did not specify the requirements for testing to verify the effectiveness of LO/TO devices and other control measures. Occurred on or about October 3, 2018.
