OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that adequate fall protection systems were not provided on elevating work platforms: Small Dry Dock: On, or about, October 19, 2018, the guardrail systems provided on the following scissor lifts were not adequate: A. Upright SL26, brown, unmarked lift with green paint on the under support: Snap bolt clips were used to secure the chain provided for the platform access opening. The snap bolt clips were not rated to withstand a load of 300 pounds and one of the snap bolts was stuck in the open position B. Upright SL26, SWL 1250: A top rail was bent and creased, a midrail was significantly corroded and a corner upright was broken.
