29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation: On or about October 12, 2018 at 36375 North Corbin Rd,, Walker, LA; the employer failed to certify that an operator of a Bobcat V723 Versa Handler powerlift had been trained and evaluated in accordance with the OSHA standard. Employee(s) use the lift to load I-beams onto a 25-foot gooseneck trailer.
