29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 17, 2018, at the jobsite located at 1109 W. Diamond Road, Salina, Kansas, an employee engaged in barrier construction activities was exposed to entanglement and amputation hazards while working in close proximity to a tractor mounted, boom type, post-hole digger and auger. The employee was exposed in that training in the recognition and avoidance of hazards associated with post hole digging activities had not been provided.
