29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about October 3, 2018, an employee operating the cut and fold machine identified as asset #9682-23 in the service area was exposed to injuries after introducing a finger into the point of operation of the equipment and the employer had not provided adequate guards during its operation.
