29 CFR 1910.179(n)(3)(ix): When two or more cranes were used to lift a load one qualified responsible person did not instruct all personnel involved in the proper positioning, rigging of the load, and the movements to be made: Shop Floor - During a two crane lift of an eighty foot long, 120,000 pound girder, a qualified person did not instruct all personnel involved in the lift in the proper movements to be made during the removal of the rigging. One of the crane operators involved in the lift was severely injured after the girder fell and struck him during the removal of the rigging.
