29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. Latrobe Specialty Metals Company, LLC - Wauseon, Ohio: On or about September 25, 2018, the employer did not ensure that employees who operate the GCR #2 cleaning machine were sufficiently trained when the company's Lockout Procedures needed to be employed (such as when cleaning, maintenance, adjustment or other lockout tasks as defined in 1910.147 requires placing the employee's hands inside the machine while the wire is still attached to the mechanized exit-side spool and has the potential to move due to the energy being present), when the Lockout Procedure did not need to be employed (such when the machine is being setup and the wire is not attached to the mechanized exit-side spool and there is not potential due to the lack of energy present, and when alternative measures which provide effective protection can be employed (such as servicing activities that are routine, repetitive, and integral to the use of the equipment, including relying on a control reliable system when appropriate.
