29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provide for frequent and regular inspections of the job sites, materials and equipment to be made by competent persons designated by the employers: a. On or about October 1, 2018, at approximately 170 feet north of Mile Marker 179 on the northbound lane of the Florida Turnpike, the employer did not ensure through frequent and regular inspections conducted by the competent person that employees were aware of and protected from 14-foot fall hazards posed by the top of a concrete cattle culvert which had the vehicle traffic guardrails removed, thereby exposing an unprotected edge.
