29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours: On July 12, 2018, the employer did not report the in-patient hospitalization of an employee within the twenty-four hour requirement. An employee slipped and fell while attempting to exit from the Powered Industrial Truck he had been operating. The fall resulted in the employee being hospitalized as a result of a broken hip. The event was eventually reported to OSHA on Sept. 25, 2018.
