29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. An employee was not protected from a (fall hazard) struck-by and crushed-by hazards. This was most recently documented on or about September 25, 2018, at the jobsite located located at 16650 E US Hwy 40 Independence MO. The employer did not provide adequate or ensure that a ladder or other means of egress was used by employees working in the excavation. An employee was working in the excavation which was 5.6 feet in depth and approximately 10.3 feet in length.
