29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees. a) Jobsite: 12" gas line inside trench - The employer did not protect the gas line as necessary to safeguard employees installing a 12" water main pipe inside the trench measuring approximately 21' in length, 4' 8" in width, and 8 ft. in depth. An employee inside the trench was using a shovel, and a 15k lb. backhoe was used to excavate soil in proximity to the gas line. Observed on or about 9/21/18.
