29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident a) Worksite, 2245 Ticonderoga Blvd, Chester Springs, PA - Dock Yard: On or about September 16, 2018 an employee died as a result of being struck in the head/neck by falling cargo freight. The employer reported the fatality on September 17, 2018, more than 8 hours after they became aware of the death.
