29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: On or about and prior to September 13, 2018, the written procedure, Operator Lockout/Tagout Procedure for Ring Debarker (Greasing or Cleaning), did not address all energy sources, specifically the gravity for the infeed and outfeed roller systems. In November 2018, the written procedure, Nicholson Ring Debarker/110 Infeed Chain/Log Deck, did not address all energy sources to include but not limited to gravity for the hydraulic rollers.
