Location: 90th East 5th St Bayonne, NJ 07002-warehouse a) The employer did not verify that a workplace hazard assessment had been conducted prior to assigning temporary employee(s) to the work site resulting with an untrained electrical pallet jack operator to become injured on or about 8/20/18. The Pro Staff Solutions Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.132(d)(1)which was contained in OSHA inspection number 1069741, citation number 1, item number 1 and was affirmed as a final order on 9/22/15 with respect to a workplace located at 4 Corporate Place Piscataway, NJ 07095.
