29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about 09/04/2018, at the worksite located at 12995 S Cleveland Ave., Fort Myers, FL, where roof skylights had been removed and covered with black felt paper, an employee was not protected from falling approximately 23 feet through a skylight opening to the floor below.
