29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Mountain Top Homes at 4273 Spruce Way in Vail CO 81657: On August 31, 2018, the exposing employer did not report the hospitalization of an injured worker. An employee sustained multiple severe injuries as a result of trench cave-in which occurred on August 31, 2018 at approximately 3:00 pm. The employer reported the incident on September 4th, 2018 to OSHA phone hotline.
