29 CFR 1910.119(e)(6): The employer did not update and revalidate by a team meeting the requirements in paragraph (e)(4) of this section, to assure that the process hazard analysis is consistent with the current process, at least every five (5) years after the completion of the initial process hazard analysis. a) 626 Lynnway, Lynn, MA On or about August 30, 2018, the process hazard analysis was not updated and revalidated, to assure that the process hazard analysis is consistent with the current process. The last process hazard analysis revalidation occurred on June 27, 2013. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
