29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) On or about August 29, 2018, and at times prior there to, an in-patient hospitalization was not reported to OSHA within 24 hours. The accident, and resulting in-patient hospitalization, took place on or about August 23, 2018. The incident was not reported to OSHA until August 27, 2018.
