OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by and crushing hazards. a. On or about August 24, 2018, in the scrap yard, an employee was working beneath a bus in order to inventory parts for salvage. The operator of a Komatsu Wheel Loader, Model Number 2VP11A, Serial Number K4680, was unaware of the employee's location and pushed the bus with the loader for it to be repositioned. The employee was crushed by the wheels of the bus as it was being moved.
