Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards of up to 9' while using a forklift with a homemade platform to perform work inside blow molding presses. a.) On or about August 23, 2018, the employer did not ensure employees were protected from fall hazards while using the home made platform on the Mitsubishi forklift. Employees are required to use the platform to perform maintenance activities such as changing head tooling inside blow molding presses, including but not limited to Sterling Press #4 and Press #7 and are exposed to falls in that employees are standing on the forks and the platform used has no form of fall protection. One or more feasible means of abatement could be to ensure the platform is approved by a Professional Engineer and that the forklift capacity plate is updated accordingly. Additionally, follow American National Standards Institute/Industrial Truck Standards Development Foundation ANSI/ITSDF B56.1-2012 Safety Standard Low Lift and High Lift Trucks, Chapter 4.17 by providing a personnel platform that complies with the design and fall restraint requirements it sets forth, and that can be secured to the lifting carriage or forks to prevent displacement.
