29 CFR 1910.410(a)(1): Each dive team member did not have the experience or training necessary to perform assigned tasks in a safe and healthful manner: Pacific Ocean off the coast of the Silver Strand, 32.39.3962N/117.09.6871 (LAT/LONG): On August 22, 2018, employee(s) were performing SCUBA diving operations to mark a sunken road line. Diver(s) were certified as open-water recreational divers by the Professional Association of Diving Instructors (PADI), but not in commercial diving operations. A diver suffered fatal injuries after he lost consciousness underwater and could not promptly be provided emergency resuscitation nor removed from the water.
