29 CFR 1926.651(l): Walkways were not provided where employees or equipment are required or permitted to cross over excavations. a) Rt 20 Millbury, MA: On or about August 20, 2018, two employees were observed climbing up a ladder out of an excavation, and then walking over a 3 foot wide gap (7.7 feet deep) at the edge of the excavation to reach the ground.
