29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: (a) On or about August 22, 2018 a recordable lost workday injury that occurred on August 9, 2018 as a result of an employee falling was not recorded on the OSHA-300 log, or equivalent form, and on an OSHA-301 Incident Report, or equivalent form, within seven calendar days of receiving information that the recordable injury occurred.
