OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed thermal burns from molten metal reaction with moisture and/or water. a. On or about August 21, 2018, in the melt department, the employer did not ensure that the refractory repair performed on a tundish ladle was moisture free before allowing furnace #3 to be tapped into the ladle; thereby exposing employees performing activities such as furnace tapping, pouring, operating crane, weighing raw materials, and any other activities in the area around furnace #3 to thermal burns from a reaction of molten metal and moisture.
